How they get in, and why it is not about cleanliness

Bed bugs travel in luggage, in a mattress bought second hand, in a box that came out of a storage unit, in a bag set down in a hotel room. A spotless house is as easy to infest as any other. Nothing about having them says anything about how a home is kept.

They hide within a few feet of where someone sleeps: the seam of the mattress, the joint of a bed frame, the screw holes of a headboard, behind a loose strip of baseboard. That is where an inspection starts.

Heat or chemical, and why prep decides the outcome

Heat treatment raises the whole room above the temperature the insects survive, in one visit, with no residue. It needs the room cleared to a specific standard and it is unforgiving about clutter, because anything that shields a harborage from the heat leaves survivors.

Chemical treatment is applied to the harborages and takes more than one visit, because it has to catch eggs that hatch after the first application. Either way, the preparation instructions are not paperwork — they are the difference between the treatment working and being repeated.

Do not throw out the furniture

A mattress dragged to the curb spreads the problem to a neighbor and takes the treatment target out of the room without removing the infestation, which is in the frame and the baseboard as well.

Do not sleep in another room either. Moving away from the harborage spreads it to a second room, and the treatment then covers twice the area.

Whether you have found anything on the mattress seam — specks, shed skins, an insect.
How many rooms have anyone sleeping in them.
Whether anything came into the house recently: travel, a second-hand mattress, a delivery from storage.
